Each winner will receive a $500 Sport Chek gift card.

The Sportchecksurvey.ca contest starts on January 1, 2019 and ends on December 31, 2020, at 11:59 p.m. MST.

The Sportcheksurvey.ca contest is divided into 12 separate entry periods.

There is one entry period per month!

The Sport Check Survey contest is open to Canadian residents who are the age of majority and older.

There are two ways to enter the Sport Check Survey Contest

Online Survey Entries.

If you have made a purchase at Sport Check stores or online at sportcheck.ca you may qualify for the contest.

next go to www.sportchecksurvey.ca to provide your feedback
begin by entering your personal information( (name, telephone number, email address
next fill in the information from your store receipt (or email confirmation) including the purchase date, store number, total amount spend and your unique survey code) or click the link in the invitation email.
follow the prompts int the survey to complete your feedback

After you have successfully completed all the steps and finished your survey you will be automatically enterered into the monthly draw.
