Each winner will get the choice to accept either
- $1000.00 Shoppers Drug Mart gift card
- OR One million (1,000,000) PC Optimum Points.
Each prize is valued at $1000.

The Shoppers Drug Mart Feedback contest begins on January 1, 2021, and closes December 31, 2021, at 11:59:59 PM (ET). A random draw will be made on the monthly draw dates as seen below.

Shoppers Drug Mart Survey Entry Rules
The Shoppers Drug Mart Feedback contest is open to legal residents of Canada who have reached the age of majority.
There are two ways to participate in the 2021 Shoppers Drug Mart Feeback survey:
Online at Surveysdm.com
- Make a store Shoppers Drug Mart store purchase and check your receipt for a survey contest invitation (there will be a survey code printed on your receipt.
- Go to www.surveysdm.com and select your preferred language (English, French or Chinese)
- then you will be prompted to enter your 15-digit survey code
- start with your survey and follow the prompts on your screen
- after completion, you will be automatically entered into the draw of the month
- if you have a PC Optimum card, please provide your PC Optimum number
Via telephone:
Alternatively, you may take the Customer Care survey by calling the telephone number listed on the Shoppers Drug Mart survey invitation
Additional information:
You must enter the survey contest within 7 days of receiving your survey invitation (the date of your receipt)
Each person/household may enter a maximum of 2 store surveys per calendar month.

Went to the SDM pharmacy to pick up prescription. Two ahead of me had problem with finding their prescription or it not being ready. Very slow. Paid for my prescription and had to ask what the price was before my benefits plan covered the cost. They did not see that I had a benefits plan and charged me the full cost. When they went to give me a credit; they had to look for the supervisor. It appears that they did not know who it was and had to go asking around until they got the pharmacist to call the supervisor. More time lost and a line up waiting to pick up their prescriptions. Ever since the changes were made a few years ago; it has been very difficult to do business with them.

i will no longer shop at Shoppers on Appleby and New street as they only allow you to
self check out, I will order online if I have to self checkout
Stop the madness bring back jobs I hope others will also stop shopping at shopper and all the other stores that will only allow you to use self checkout

the Williams Lake , B.C. Shoppers Drug Mart has the best staff and they are super friendly and extremely helpful. Gail, Irene, Pam, and Tamara are the best. Also the pharmacy is amazing and extremely helpful with all my needs as i am going through chemo and had a very difficult time the past year. Most stores i dread going to but not shoppers drug mart. keep up the great work ladies. thank you.
