The Lifetouch Customer Satisfaction Survey Sweepstakes runs from January 1 through December 31, 2021.
One winner will be selected in random drawing each month of the contest period, will be notified via email and MUST respond within the time stated in the notice.

You must be 18 years of age or older to participate.

Have you recently purchased a photography package through Lifetouch? If so you may have received an invitation via email or your receipt to complete a survey.
- Go to www.lifetouchsurvey.com
- Follow the on-screen instructions to complete the survey form and submit your entry.
OR
- Write your complete name, mailing address, phone number, and email address, on a postcard or a card that is no smaller than 3 ½ x 5 inches, and no larger than 4 ¼ x 6 inches
- Mail it to: Lifetouch Sweepstakes, PO Box 957, Macedon, NY 14502-0957.
