First, purchase any Jones Soda Glass Bottles from the following Black/White Caps from Jones Soda/Zilch Glass Bottles or Black 7-Select by Jones Bottle Caps)
keep the bottle caps
then visit the offers page at www.capsforgear.com where you will find the claim form
select either US or Canadian and print out the form
then complete the form and select your chosen item
Mail in the original entry form AND your bottle caps
