Prize Details

CMG Strategy Co. is giving away TWENTY FOUR Prizes – 1 per Entry Period from the US Pool and 1 per Entry Period from the Canada Pool.

The Twenty Four Prizes, each consists of fifty-two Chipotle Burrito Cards worth up to $10 each card, which can be redeemed at any participating Chipotle Restaurant. The value of each Prize is $520 USD / $675.42 CAD.

The Chipotle Feedback Customer Satisfaction Survey Sweepstakes is open to residents of the fifty United States, the District of Columbia, and Canada (excluding Quebec). You must be 13 years of age or older to participate.

To participate in the Chipotle Feedback Customer Satisfaction Survey Sweepstakes

Make a purchase of any food or beverage at a participating Chipotle Mexican Grill restaurant. Each Purchase receipt will include a unique code.
Go to www.chipotlefeedback.com
there begin by selecting how you made a purchase (in Restaurant with the receipt, digital code with online order, delivery)
now enter your 20-digit survey code
next, proceed and follow the instructions to complete to answer questions regarding the service you received and the quality of your Chipotles Restaurant
and submit the survey form.

You also have the choice to enter without making a purchase, in that case select the “I do not have a survey invite” option to enter

On 3½”x5″ card, hand-write your full name, email address, street address, city, state, ZIP code, telephone number, and date of birth
